POSITION DESCRIPTION

POSITION TITLE: REGISTERED NURSE

DEPARTMENT: NURSING

REPORTS TO: DIRECTOR OF NURSING

POSITION SUMMARY: Functions as the SCACADHC nurse, completing all nursing responsibilities under the direction and supervision of the Director of Nursing. In the absence of the Director of Nursing serves as the Director of Nursing for the Adult Day Health Center.

SPECIFIC DUTIES:

1. In the absence of the Director of Nursing serves as the Director of Nursing for

the Adult Day Health Center.

Administer and record medications for members as directed by the designated

physician’s order.

Update medications and medical information (change of medical status, etc.)

including medical records, under the supervision of the Director of Nursing,

the member’s (and/or SCACADHC’s medical consultant) and, in concert with member’s caregivers.

Maintain adequate supplies of medication from caregivers as well as general

medications and nursing supplies for the Program.

Monitor and record member’s blood pressures, weights monthly.

Monitor member’s blood sugars as required.

In addition to non-emergency nursing services, provide emergency nursing assistance to members as needed.

Assist at meal time in feeding members as needed.

Monitor and record intake of members who have poor appetites or eating problems as per the request of member’s physician, caregiver or Director of Nursing.

Maintain adequate inventory of toileting and incontinence supplies for the Program.

Conduct health teaching with members, and caregivers as directed by the Director of Nursing.

Assist with monitoring member’s who wander.

Assist Activities Director and staff, where appropriate, with member’s requiring individualized attention in recreational activities.

Work with Personal Care Aide with education and assistance in providing assistance in activities of daily living such as walking, eating, toileting, grooming, and supervision of personal hygiene of members.Company Description
