SOME (So Others Might Eat) provides material aid and comfort to our vulnerable neighbors in the District of Columbia, helping them to break the cycle of poverty and homelessness through programs and services that save lives, improve lives, and help to transform the lives of individuals and families, their communities, and the systems and structures that affect them. Our more than 30 programs throughout the district provide comprehensive services that lead to mental and physical health, continued sobriety, job training, living-wage employment, and affordable housing. We meet immediate needs with food, clothing, and healthcare and offer the tools one needs to live with hope, dignity, and greater independence.

Compensation:

We offer our employees a competitive compensation and benefits package that reflects our organizational culture, mission, and core values. The hourly range for this position is $23.54 to $25.00 and is commensurate with experience.

Position Description: The Family Service Case Manager - Youth Service Coordinator provides case management and support services for adults and children living in Family Housing and also, develops and coordinates programming for youth on life skills and education, and personal development.

Covid-19 Considerations: In alignment with DC Health, COVID-19 vaccinations and boosters are mandated for personnel working in SOME’s healthcare-designated programs, which currently include Medical, Dental, Population Health, Behavioral Health Services (BHS), Exodus House, Jordan House, Kirwan House, Keuhner Place, Leland Place, and Mary Claire House. Qualifications:

Required: High School Diploma/GED; 5-8 years experience in social services; case management experience and program development experience.

Preferred: Bachelor’s degree; 3-5 years experience in social services; 2 + yrs. case management experience and program development experience; experience providing services to low-income families. Fluent in Spanish and English (Spring Road only) Driver's License and access to a vehicle.

Required License/Certification: CPR/First-Aid, CPR for Infants.

Schedule: Monday-Friday, 8:00 am-4:30 pm morning early shift, 11:30 am to 8:00 pm evening shifts. (Employee will choose whether they'd like to do 2 or 3 evening shifts per week)

Knowledge, Skills & Abilities Required:

Knowledge of principles, methods, and procedures, for motivational enhancement interviewing and for career counseling and guidance. Being aware of others reactions and understanding why they react as they do.

Knowledge of principles and processes for providing customer and individualized services.This includes understanding resident needs, meeting quality standards for services, evaluation of satisfaction.

Knowledge of family/group/individual/ behavior and dynamics and sensitive to diverse cultural backgrounds.

Ability to write effective SMART goal plans and document progress

Ability to communicate effectively in writing and verbally as appropriate for the needs of the audience. Talking to others to convey information effectively.

Ability to educate and teach residents how to access and utilize community resources

Ability to evaluate the level of care and implement service appropriately

Ability to learn, comprehend and apply clinical interventions

Ability to perform assessments

Ability to lead and facilitate workshops and life skills trainings

Ability to creatively and effectively use problem-solving skills to draw conclusions, evaluate options and implement solutions.

Ability to effectively organize and prioritize work tasks with effective time management skills

Ability to engage and develop strong professional relationships

Understanding of issues of trauma and mental health

Ability to work independently and in a team environment

Ability to establish and adhere to strong professional boundaries

proficient with MS Office, including Word, Excel, and Outlook

Expected Contributions:

Case Management:

Responsible for providing case management services utilizing the strength-based perspective and integrating motivational enhancement theory.

Collaborate with families and individuals to develop goals plans and monitor and report progress

Collaborate follow-up, monitor and track service outcomes making necessary adjustments to services based on progress and need.

Responsible for identifying, educating, advocating, and teaching residents how to access and utilize community resources.

Responsible for implementing clinical interventions

Meet with families/individuals for case management, at regular intervals

Conduct home visits based on program requirements or as needed and assess environmental concerns, family dynamics and any other issues.

Perform intakes and annual assessments and incorporate findings into case management services

Responsible for participating in the integration of care with outside service providers at least twice a year.

Responsible for performing school visits and interfacing with education support staff to ensure children's educational needs are met.

Maintain progress notes and program charts using appropriate documentation standards.

Program Activity Development:

Develop and coordinate activities in areas of improvement for youth on life skills and education and personal development.

Assist with the development of SOME Place for Kids after school program focusing on education, social development, tutoring, mentoring, reading, arts, and culture.

Responsible for promoting, marketing and ensuring attendance for all program activities and initiatives

Identify resources and establish partnerships to assist in activities

Evaluate program effectiveness and document resident feedback

Develop and submit quarterly community events calendar and family/adult/youth activity plans

Assist with program operations, as needed.
