Hospice Referral Coordinator

A health services network in New York City is currently seeking a licensed Registered Nurse (RN) to join their staff in Manhattan as a Hospice Referral Coordinator.

Responsibilities:

The Hospice Referral Coordinator (RN) will be responsible for:

Evaluating and establishing a plan of care for patients referred for home health services

Facilitating admission of referrals upon discharge from the hospital to a home care program that meets the needs of the patient and family members

Developing and maintaining positive relationships with Physicians, Social Workers, Case Managers and Discharge Planners

Performing other duties, as needed

Qualifications:

2+ years of experience in Home Care Nursing

Graduate of an accredited School of Nursing

NYS Registered Nurse (RN) license

Proficiency in Physical Assessment and Interviewing skills

Solid assessment, clinical, and documentation

Great interpersonal skills

Excellent communication skills (written and verbal)

Strong attention to detail

Highly organized

Desired Skills:

Bachelor's Degree in Nursing
