HIV/AIDS Case Manager WANTED

Location: Wilkes-Barre, PA

Compensation: $21.27 per hour

Status: Full-Time (37.5 hrs/wk)

Job Description: Conducts intakes and coordinates the assessment/treatment plan process and follow-up for each client; serves as an advocate and liaison for and on behalf of clients; assists in the carrying outreach and health education/risk reduction efforts as they relate to agency’s funding expectation.

Essential Duties and Responsibilities:

1. Conducts client in-take process including completing paperwork and assessing the client’s initial needs, required paperwork/identification/etc. in a timely manner.

2. Assures that all participants in case management program complete an individualized service plan on regular basis.

3. Evaluates, facilitates and monitors services and clients’ progress toward meeting goals/objectives. Documents such progress in client file and/or computer system.

4. Assists clients in navigating their health care needs, including, but not limited to, assisting them in finding permanent/stable housing, signing up for government benefits, obtaining and maintain comprehensive health insurance and/or SPBP coverage, scheduling and attending medical appointments, obtaining medical transportation, etc.

5. Conducts outreach and health education/risk reduction for the Minority AIDS Initiative Program.

Requirements

Education

· A licensed Registered Nurse or bachelor’s degree in human service field from an accredited college or university; or unrelated bachelor’s degree and two years’ experience in case management/ social work.

Experience

· 1-3 years of experience in case management preferred

Knowledge, Skills and Abilities

· Ability to read and speak in Spanish preferred, but not required

· Knowledge of local health and social services preferred

· Knowledge of HIV transmission and current HIV medications helpful

· Ability to work independently as well as a member of a group/team.

· Ability to communicate effectively with persons of diverse social, economic, cultural, age, sexual and racial backgrounds.

· Maintain required job knowledge, skills and core professional competencies.

· Ability to organize work, set priorities, exercise independent judgement and to demonstrate flexibility in balancing the needs of the program and clients.

· Ability to input and maintain accurate records, perform research and compile data.

· Ability to attend departmental and all-staff meetings as directed, staying informed of ongoing changes in the department and organization.

· Ability to operate agency and department specific software systems, standard office equipment (machines, tools and devices) used in performing essential functions including computer related equipment, copiers and telephones.

Physical Requirements or Licenses

· Ability to lift and move up to 35 pounds.

· Valid PA driver’s license required. Travel throughout the 6-county region and beyond is a regular part of this position.

Interviews will be on a rolling basis until the position is filled. Initial applicants will be reviewed beginning on March 25, 2022. Those candidates selected for further consideration will be contacted with five business days of the initial review process to schedule an interview.
