Master Social Worker - MSW

JOIN TEAM TRILOGY:

Trilogy Health Services, an innovative senior living company is looking for a Master Social Worker to join the Dialysis team at our health campus. The goal of our Master Social Worker is to promote social functioning, psychological adjustment, and overall well-being in each renal patient.

Here are a few of the responsibilities of the Master Social Worker:

Assessing the patient’s financial, medical, social, and familial needs.

Meeting the patient’s needs through direct supportive counseling or through referrals to other agencies.

Recommending changes in treatment based on the patient’s current psychosocial needs.

Providing case work and group work services to patients and their families in dealing with the special problems associated with end stage renal disease.

As a member of a Home Dialysis Training Unit, participate in periodic home visits

WHAT WE'RE LOOKING FOR:

Holds a master’s degree in social work with a specialization in clinical practice from a school of social work accredited by the Council on Social

Has served at least two years as a social worker, one year of which was in a dialysis unit or transplantation program prior to September 1, 1976, and has established a consultative relationship with a social worker who qualifies under § 494.140 (d)(1

The social worker must meet the licensure requirements in the state of practice.

Worked at least two years as a social worker, one year of which was in a dialysis facility or transplantation program.
