Salary: $24 - $27hr

Job Description:

The Care Coordinator will provide recovery focused services related to the safe and appropriate administration of medical treatment (including medications) as prescribed by the physician. They are responsible to provide safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care. Shifts available: 8:00 am – 8:30 pm, 8:00 pm – 8:30 am

Only shifts available is Tues-Thrs, 12 hour shifts. Ideal one person will do all 3 shifts

Job Duties:

Ability to maintain confidentiality of patients' medical and personal information

Respond to emergencies and initiate action as required

Observation, documentation, and record keeping of client’s health status

Be familiar with common detox assessments, CIWA and COWS preferred

Ensures client compliance with facility rules and guidelines

Skilled in case management, time management and crisis intervention

Follows HIPAA standards/laws in all aspects of their duties

Provide care and assist in the welcoming, admission and discharge process for clients

Collects data relevant to person served care

Develops and implements nursing care plans

Transcribes physician orders accurately and in a timely manner

Observes client self-administer medications as prescribed

Receives and presents change of shift report as assigned

Adheres to medication management policies and procedures

Monitor and itemize OTC medications/supplies and notify management of all medications/supplies to be ordered.

During downtime will assist with household chores and help with the overall functioning of the facility (general cleanliness and safety of the house) which includes but is not limited to laundry (washing, folding, putting away), bathroom inventory (stocking toilet paper, paper towels, wiping down counters, empty trash, cleaning mirrors), food handling (daily check of any expired food items from the fridge & ensuring that all food items are labeled with date of expiration), and maintain cleanliness of rooms and common areas throughout the facility (sweeping and mopping).

Assists with conducting TB, Pharmacy, and store runs

Notifies the Medical Director and Supervisor of any changes in behavior and/or condition

Sanitizing all stations and areas of the facility

Minimum Requirements and Qualifications:

Current license/certification in the State of California (required)

Current CPR/First Aid (required)

Minimum 1 year of nursing experience

Preferred 1+ years experience in chemical dependency

Proficient knowledge of chemical dependency and psychological disorders preferred

Ability to communicate and work effectively with diversity (required)

Knowledge of detoxification protocol and related care including symptoms and medications preferred

Ability to accurately record information and prepare reports

Strong ability to establish and maintain effective working relationships with the staff, patients, patient's families and authorized consents

Willingness to be a team player and assist with nursing tasks
