The Nurse Practitioner focuses on providing general care to patients of all ages and attend to the patient’s needs with utmost patience, care, compassion and understanding. 100% of the position is travel spent going to patient's residence and providing treatment. A company vehicle will be provided.

Key Responsibilities

• Conduct in-home assessments and evaluation to certify home health care, check vitals, including past medical history, review of symptoms, physical examination and medication review.

• Obtain consents at the time of visit and submit to intake team within 24 hours

• Complete EMR documentation within 24 hours of patient visit

• Review current condition of patient and determine treatment option and initiate plan of care

• Facilitate referrals to other healthcare professionals and medical facilities, as needed

• Write prescription and order for medical tests (as appropriate)

• Provide patient education

• Provides consistent reasonable and prudent practice of advanced nursing

• Participate in bi-annual ride-along with Medical Director for practitioner and process improvement evaluation

• Communication between NP and scheduling office will be emailed, signed, or documented for all patients.

• Discuss any issues or concerns with Medical Director

• Dress must be professional, i.e. clean scrubs, clean lab coat, and name tag displayed.

• Demonstrate promptness, courteousness, and professional demeanor

• Report patient issues immediately. NPs are not required to be in an atmosphere where they do not feel safe or respected.

• Annual performance review with Medical Director

• Attendance required at Nurse Practitioner provider meetings

Required Qualifications

• Holds current license with Texas Board of Nursing

• Holds current Basic Life Support certification

• Ability to obtain DEA licensure

• EMR experience

• Certification from either the ANCC, AANP or AACN as Adult, Geriatric, Family or Acute Care NP

• 1+ year of clinical experience in highest level of education OR 3+ years of experience as a Registered Nurse in Acute Care, ED, Home Health or Hospice experience.

• Demonstrated ability to communicate, collaborate, problem-solve, organize, and plan

• Ability to build rapport with patients

• Excellent written and verbal communication skills

• Ability to spend at least 1 hour with patient in their home, which may be in remote areas

• Comply with all HIPAA regulations and maintain security of protected health information (PHI)

• Ability to lift 20 pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations

• Driver’s license required, good driving record; transportation coordinated after approximately 90 days.

Preferred Qualifications

• ACLS certification

• Experience in Gerontology, Cardiology, Internal Medicine and/or Endocrinology

• Home care or home visit experience

• Excellent administrative and organizational skills and the ability to effectively communicate with patients and their family members (as appropriate)

• Computer literate and able to navigate using Global Positioning System (GPS)
