What You'll Do

Provide direct patient care services to hospice patients and support the family after normal business hours

Evaluate patient and family needs and assist in the execution of the patients plan of care

Document and maintain clinical records

Attend staff meetings and participate in Quality Improvement initiatives

JOB REQUIREMENTS & QUALIFICATIONS:

Maintains an active nursing license that is in good standing in the state performing job duties.

Maintains an active CPR certification.

At least 2-5 years of experience in the healthcare management, hospice, or palliative care and/or training in end-of-life care, preferred.

Proficient in MS Office applications and ability to learn department and job-specific software systems.

Demonstrate organizational skills. Demonstrate effective verbal and written communication skills. Demonstrate analytical skills when problem-solving. Demonstrate high attention to detail and a high degree of accuracy.
