Responsible for providing activities and interactions that enable an individual at clear risk of hospitalization or who has been discharged form inpatient treatment to maintain an adjustment to living in the community.

PRIMARY DUTIES AND RESPONSIBILITIES

Responsible for assessing the client's status including personal strengths, progress, problems and needs,

Participating in the formal assessment, development and review of the client's individualized treatment plan which will focus on the client's unique strengths and needs and encompass all areas of the client's life.

Monitors the clients participation and progress in an organized treatment programs to assure the planned provision of services according to the client's plan of care is being followed and/or revised as appropriate.

Provides individual assistance to all clients as a twenty-four (24) hour resource in accessing a variety of public services including financial, medical assistance, housing and emergencies. Provides crisis follow-up to clients calling crisis line within 24 hours.

Perform outreach and engagement services to clients newly assigned or to those who have been inactive from treatment.

Intercedes on behalf of individual clients within the community at large to assist the client in achieving and maintaining their community adjustment.

Links clients and families to internal supports and services to include Psychosocial Rehabilitation, Nursing Services (medication education, metabolic screenings, education on nutrition and other disease management initiative and AIMS as appropriate).

Maintains contact with clients who are hospitalized and participates in discharge planning of clients receiving services form an inpatient facility.

Trains clients in daily living skills such as housekeeping, cooking, personal grooming, and works with them on basic behavior modification strategies.

Assists individual clients in creating personal support systems that include working with family members, legal guardians or significant others regarding the needs and abilities of an identified client.

Participates in multi-disciplinary teams for staffing clients.

Completes necessary documentation on all aspects of community support as it applies to individual clients including case opening, referrals, progress notes, discharge planning and case closure.

Participates in staff training as a team and on an individualized basis to learn skills to include: interviewing, assessment, ability to establish rapport and maintain relationships, give direct treatment planning, knowledge of promoting positive behaviors, self-direction, knowledge about mental illness, etc. Completes annual training as requires

Maintaining patient charts and assuring the standards and guidelines in quality care are followed:

Maintain treatment plan and assessment with all the necessary elements;

Make sure that a physician's signature of another qualified staff is present on the treatment plan and assessment;

Make sure the treatment plan and assessment are done within the required timelines and include the required elements and signatures;

Write progress note that contain interventions related to the treatment plan;

Ensures the service has been billed to the appropriate payer source.

Serves as advocate for families in family court, IEP meeting, FST and TDM meetings with the State of Missouri Department of Social Services, or with other agencies.

Maintains quarterly productivity standard to ensure majority of working time is spent delivering direct services and completes all documentation within the required time frames.

MINIMUM EDUCATION, TRAINING AND EXPERIENCE:

Bachelor's degree in Social Work, Psychology, Nursing or related field from and accredited school.

Equivalent experience will be submitted on the basis of one year of experience for each year of required educational experience.

CERTIFICATION OR LICENSE REQUIRED:

As required by the field.

ADDITIONAL REQUIREMENTS

Valid driver license, dependable automobile with valid registration, and active automobile insurance.
