Full Time/ In-office

Our team is continuing to grow and we’re looking to add a Medical Practitioner to help create successful care plans for our patients and ensure a path to better health outcomes. This role will work with, and lead, our Patient Care Managers and Patient Navigators to keep the patient's best interest and health in mind at all times. Working with our VP & National Director of Medical Services, they will use their judgment and the practices created at AndHealth to create comprehensive care plans.

Key Deliverables:

Create an exceptional experience for patients enrolled in AndHealth’s chronic diseases programs when delivering care virtually and in-person

Work alongside working group team members to research, document and produce protocols that address AndHealth’s Therapeutic Areas.

Help to create medical practitioner facing documents that simplify the delivery and decision making process used in the delivery of AndHealth’s Therapeutic Areas.

What you’ll do in the role:

Deliver virtual and occasionally in-person specialty care for patients with various medical conditions currently focusing on Auto-immune conditions and migraine with new conditions to be added

Create personalized treatment plans for patients

See patients directly and work with them to implement their care plan as needed.

Collaborate with program development leads

Review health coach interactions with patients and provide feedback

Participate in assessment of health coach care provision quality

Assess medication readiness/eligibility, and manage medications as appropriate for patients.

Order and interpret laboratory testing as needed.

Review and oversee patient progress; engage with coaches to coordinate care for patients; and review progress during clinical rounds and serve as clinical escalation point for coaches outside of regularly scheduled medical appointments

Triage patient calls when appropriate

Cover messages and calls from patients

Utilize a data-driven approach to reflect back to patients and be held accountable to improving patient outcomes

Assist with the use of wearable and lab data in adjusting patient care plans

Deliver care consistent with AndHealth’s values and brand

Collaborate with VP of Operations to improve program operations

Create and collaborate with the care team on the delivery of a care plan that provides recommendations regarding nutrition, lifestyle modifications and sleep that will move the patient closer to achieving their short and long-term goals.

Select and direct the ordering of supplements for patients.

Document each patient visit according to the training provided by AndHealth, upon completion of a patient’s visit.

Participate in scheduled Grand Rounds/ Huddle discussions pertaining to decisions and recommendations that are used in creating a patient’s care plan.

Maintain CME required to maintain medical licensure.

Use AndHealth approved technology to communicate securely with each patient.
