PURPOSE OF POSITION: To provide case management services and transitional housing support which assure the applicants and their families that recommendations about admissions to Life Changers, referrals to alternate resources, and the initial clinical assessment report are based on accurate, thorough and appropriate psycho-educational information. The Case Manager will ensure the implementation of each client s Individual Service Plan (ISP), that the support team have necessary resources for the implementation of clients treatment goals and to monitor all program communication, incident reports, phone/written communication to Program Manager, families and to external bodies, as appropriate. Services include but are not limited to the following: Crisis Intervention, Crisis Stabilization, Intensive In-Home, Mental Health Support, Intensive Community Treatment, Psychosocial Rehabilitation and Substance Abuse Services. Specifically, but not exclusively, Case Manager will oversee client services and supervise the Counselor as he/she provides assistance with activities of daily living, functional living skills, fulfill the goals and objectives listed in the Comprehensive Assessment and Individualized Service Plan, and comply with all required activities covered by Medicaid and Licensing to meet and/or exceed eligibility requirements. ESSENTIAL QUALIFICATIONS: 1. Licensed in Virginia as a Clinical Social Worker through the Virginia Board of Social Work or licensed in Virginia as a Clinical Professional Counselor through the Virginia Board of Counseling. Or, registered, license eligible in the state of Virginia. 2. Or, any combination of education and experience equivalent to graduation from an accredited college or university with a Bachelor s degree in Psychology, Social Work, Counseling, Human Services, or closely related field of study. Three years of professional clinical experience. 3. Must maintain standing as a Qualified Mental Health Professional as required by the Department of Medical Assistance Services (DMAS) and the Department of Behavioral Health and Developmental Services (DBHDS). 4. Experience working in a supervisory capacity with the human services field is preferred. 5. Experience with children in a community or residential setting and/or adults requiring clinical services based on community based programs. 6. Excellent communication skills as well as an ability to work with a variety of people in providing the highest level of therapeutic counseling services. 7. Regular and predictable attendance and regular meet client counseling requirements for clinical services. 8. Must be able to successfully pass State, Federal, and Central Registry background reviews. 9. A valid Virginia s driver s license, good driving record, and with personal auto insurance. 10. Knowledge of health and safety precautions. Certification in basic first-aid, CPR, crisis intervention, and medicine administration. ESSENTIAL FUNCTIONS: While the most essential functions are outlined below, responsibilities include but are not inclusive of tasks which the Case Manager shall provide. Supervision 1. Supervises and monitors each Counselor to ensure the delivery of therapeutic services for clients, as appropriate. 2. Provides Monthly Supervision to monitor performance for Counselors assigned. Participates in all supervisory meetings appropriate for job function. 3. Provides support to the Counselors by ensuring training goals are implemented and evaluated and material and supplies are available to effectively work with the clients. Identifies training needs and provides or arranges training for staff in program policies and procedures. Tracks participation of staff in training necessary for continued licensure or to ensure staff members are properly trained. 4. Supports and implements the policy and philosophy of the agency and program. Case Management & Documentation 1. Conducts clinical interview
