POSITION SUMMARY: The Direct Care Aide is responsible for supporting, educating and assisting individuals in all areas of daily living. The duties may include but are not limited to: community orientation and inclusion; health care; social, recreational, vocational and educational programs. Direct Care Aide also assists client with behavior management, communicating and interacting with outside support services and friends and family. ESSENTIAL FUNCTIONS: 1. Provide general attention to clients non-medical needs in accordance with an established Plan of Care. Monitor clients throughout work shift. Read and carefully review all shift logs. 2. Assist the client with personal care activities including, but not limited to: bathing, dressing, toileting, grooming, oral hygiene, ambulation/transfers, laundry and feeding. 3. Provide companionship in the form of talking and active listening, reading aloud, providing social and emotional support. 4. Housekeeping of the facility and general tasks to maintain a clean, safe and healthy living environment. 5. Meal preparation, grocery shopping, and errands. 6. Accompanies client and provides incidental transportation to doctor s appointments, community trips, bank, etc. as required. 7. Administer medication to client according to medication management policies or assist clients in self-administering medications. 8. Observes and reports any change in the client s mental, physical or emotional condition to immediate supervisor in a timely manner. 9. Establishes and maintains effective communication and a professional relationship with clients, family members and co-workers. 10. Implements social activities for group participation. Initiates client involvement in group activities. 11. Completes required documentation of services delivered and submits it to the office in a timely manner. 12. Maintains strict confidentiality regarding pertinent client information. 13. Participates in all in-service training, required certification trainings, and staff meetings as requested by the immediate supervisor. 14. Conduct facility required drills and safety precaution exercises. 15. All other duties as assigned. A. EDUCATION AND FORMAL TRAINING: High School Diploma or equivalent with certification and training in personal caregiving. CPR and First Aid (adult) training is required. Medication Administration Certification is preferred. Must possess valid Virginia Driver s license (reviewed annually) with good standing driving record (reviewed randomly). B. WORK EXPERIENCE: At least one year (up to and including) of professional adult caregiving experience. Solid work record that demonstrates excellent attendance and exceptional performance in providing care for adults. C. KNOWLEDGE, SKILLS, AND ABILITIES: 1. Excellent interpersonal skills and social perceptiveness. Ability to properly assess, respect, and monitor those served to insure their health and safety. 2. Display a positive, flexible, and cooperative attitude towards clients, supervisors, and co-workers. 3. Excellent manners and general courtesies, tact, and maturity. Professional grooming and personal hygiene. 4. Good homemaking skills and the ability to function in a home-like setting with minimal direct supervision. 5. Strong organizational skills and multi-task abilities in overseeing multiple individuals. 6. Flexibility in working 12 hour shifts, weekends, holidays, and call-in shifts. 7. Actively seeks ways to best support clients. 8. Evidence of the practice of a high level of confidentiality and professionalism. 9. Ability to interact with clients and support clients with disabilities and/or behavioral health challenges. 10. Ability to read, comprehend, analyze and interpret information provided in training manuals, memos, handbooks, and logbooks. 11. Ability to communicate effectively, both orally and in writing. 12.
