Health Care Management Nurse

Annual Salary:

Level I: - Y15A - $58,055.13/ Per Year + Full State Benefits Package

Level II: - Y15B - $62,409.27/ Per Year + Full State Benefits Package

Travel is occasional - Must possess a valid driver's license and must maintain required car insurance.

Minimum Qualifications:

Level I:

Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)

Two years of professional nursing experience

Level II:

Possession of a valid permanent Oklahoma license as a registered nurse as approved by the Oklahoma Board of Nursing or Registered Nurse Licensure Compact (eNLC)

Three years of professional nursing experience

Job Responsibilities

This is the specialist level of the job family where employees are assigned responsibilities adherence to established health care standards and the effectiveness of health management plans and/or programs and recommending changes or revisions.

At this level, employees have a high degree of independence in planning and completing work assignments and function as the lead person on some special projects.

The HCMN RN role is one of support to assist the assigned consumer's CM in the following: Complete a Health Review with recommendations annually on all assigned consumers prior to the yearly IP meeting. Health Review is placed in CCM documents. Complete Medical Screen in CCM (which includes the Medical History) minimum of yearly and imports it into CCM documents.

Identify direct care staff health training needs to care for the consumer.

Assist in identifying professionals that are needed (primary care physicians, dentist, psychiatrist, and nursing agencies). Providing options to consumer, CM and team of medical resources to meet the consumer's medical needs within 30 days following request with exceptions of emergencies.

Develop a Nursing Service Support Plan (NSSP) if indicated.

Consult with case manager and consumer team yearly as to the need of a pharmacological evaluation request.

Attend IT's and IP's of consumers when health issues are being discussed, training needs, and assist with health outcomes when needed.

Participate in review of consumer plans for Intrusive procedures.

Communicate with hospital when consumer is hospitalized twice a week to obtain health status. Provides this information to CM and Area Nurse Manager. Participates in hospital discharge planning.
