Hilton East Assisted Living currently has multiple Full-time openings for a Home Health Aides at our assisted living facility located in Hilton, NY.

SHIFT DETAILS:

Full-Time:

Sunday, Monday, Thursday - Saturday; Day Shift

Sunday - Thursday; Evening Shift

Tuesday - Saturday; Evening Shift

Friday & Saturday; Evening Shift

Friday & Saturday; Overnight

Sunday - Thursday; Overnight

Responsibilities and Functions

Works under the supervision of the Registered Nurse and/or Licensed Practical Nurse in accordance with the New York State Department of Health regulations.

Provides clients and/or residents with assistance with ADL’s and housekeeping tasks, as dictated by the Nurse and/or Patient Care Plan

Able to follow the Nursing Care Plan to complete the daily activities of the client and/or resident, as well as completion of the aide activity sheet.

Complete all documentation in a neat and timely matter

Observe the client and/or resident, and report changes appropriately to RN or LPN

Attend required In-service training and HHA meetings.

Must complete 12 In services per year to stay certified.

Performs tasks and duties assigned or requested by shift supervisor.

Exhibits necessary interpersonal skills to work well with clients and families.

Work hours as scheduled by supervisor.

Requirements:

High School Graduate or GED Equivalent

Must be 18 years of age

Graduate of New York State Department of Health approved HHA Training Program

Motivation and ability to work independently

Demonstrates dependability

Able to communicate effectively, both orally and in writing

Able to maintain confidentiality at all times

Adherence to policies and procedures
