The Employee’s duties shall include, but not be limited to the following:

Scope:

Works with all departments as well as outside agencies.

General Function:

To oversee the compliance of both the administrative and clinical operations within the Assisted Living Community

Duties and Responsibilities:

Assure completion of functional and clinical evaluations on a quarterly basis and as indicated by a change in resident condition;

Work in coordination with the Admissions Director to review potential admissions to the Assisted Living/Memory Care Community for appropriate level of care;

Conduct home visits to evaluate potential admissions for appropriate level of care;

Coordinate with applicable case managers and other entities to ensure coordination of care;

Coordinate with Business office all aspects of AL waiver Medicaid process; communication with AL Waiver Case managers re eligibility, tier changes; patient liability and any other financial situations that arise with service plans necessary for financial approval

Manage CTS finds- ordering of items, management of the process from initiation thru set up

Assist with completion of the admission paperwork; ensuring documentation is completed timely;

Provide oversight and review of medication management compliance as determined by physician;

Coordinate and ensure documentation reflects resident changes in condition changes;

Ensure medical record documentation accurately reflect the resident’s level of care and services provided;

Welcome and orient new residents as well as their families to the community;

Facilitate responses to resident/family questions and coordinate resolution for resident/family concerns;

Provide supervision and oversight of Assisted Living operations and staff;

Scheduling of Personal Care Attendant staff

Interview and hire Assisted Living staff under the direction of the Director of Nursing;

Coordinate and participate in the training of new Assisted Living staff;

Coordinate Activity calendar with Activities Director to ensure that activities reflect the preferences of the residents;

Assure community is clean, neat and orderly; working in coordination with Director of Housekeeping and Maintenance

Assist when needed as personal qualifications or licensure allow: dispensing medication; cleaning the personal care wing hallways and resident rooms, serving the resident meals, providing activities, and other duties needed to ensure the wing is running effectively and efficiently;

Participate in on call rotation to answer questions from staff or for finding a replacement if there is a call-off;

Other assigned duties by the Director of Nursing or Administrator;

Providing monthly Wellness Director Report to Director of Nursing and Business office manager;

Provides quarterly QA report to Director of Nursing and presents findings to QA committee;

Qualifications:

Must be either a Licensed Practical Nurse/Social worker/Activities Director and have a current license for the state of Ohio if applicable. One-year experience preferred in the geriatric setting. Must be organized and capable of multitasking.

Because of close contact with the residents, this position has been determined to have potential for exposure to blood borne pathogens. Country Club Rehabilitation Center will provide training, information and equipment to minimize the potential risk.

It is a requirement of this position that the individual comply with all privacy policies and procedures of the Provider and protect the privacy of Personal Health Information as required by law.
