Position Summary

Fora Health is seeking a compassionate, trauma-informed Psychiatric Mental Health Nurse Practitioner to join our multidisciplinary team in providing medications for addiction treatment as well as mental health medication management services to our dual diagnosis population. In this role, you will have the opportunity to work closely with the outpatient program, withdrawal management, and adult residential to provide medication management for mental health and substance use disorders. You will work closely with a team of addiction counselors, mental health providers, physicians, nurse practitioners, and support staff to support our patients as they navigate treatment for substance use disorders. This position offers an opportunity to learn and grow as you meaningfully engage with practices around harm reduction, trauma informed care and evidence-based best practices.

This role will be supporting our growing outpatient services programs at Fora, including intensive outpatient SUD treatment services, outpatient SUD treatment, and Medications supporting Addiction Treatment only services. The PMHNP will also provide support to residential patients, including psychiatric evals and follow up appointments to support coverage, including PTO for other providers.

Provides Medications for Addiction Treatment (MAT) Services

Coordinates with withdrawal management, outpatient, and residential treatment programs at Fora to provide MAT services

Completes a Biopsychosocial assessment for patients referred to MAT

Identifies a treatment plan specific to patients’ substance use pattern consistent with evidence-based practice and with patients’ goals of care

Practices harm reduction principles in all aspects of patient care

Refers patients for any acute or chronic medical needs to primary or specialty care services

Independently performs telehealth visits on time and within allotted time

Performs injections of parenteral medications used to treat substance use disorders (i.e. sublocade, brixadi and vivitrol) Provides psychiatric medication management

Completes psychiatric evaluations with dual diagnosis patients who are referred for assessment, prioritizing patients according to their need and acuity

Makes recommendations for psychiatric medication management based on evidence-based practices and patient goals

Collaborates and consults with treatment team about psychiatric status of clients

Educates patients on general health guidelines and preventative measures to improve over-all health and decrease risk factors

Communicates accurate medication orders to the pharmacy

Orders and evaluates diagnostic tests when appropriate

Enters accurate medication and provider orders into the EMR

Supports patients in following through with referrals to other appropriate resources to meet mental health needs and exchanges relevant information with agency or professional to who the referral is being made

Collaborates with outside providers as needed in order to optimize care

Document assessment, planning, implementation and evaluation of interventions in the patient records Consults with admissions team about psychiatric stability for potential residential referrals

Leadership/Administration

Collaborates closely with the Medical Manager of 3.7 services to provide leadership to the 3.7 clinical team,

Assumes leadership of the team in the Manager’s absence.

Education

The PMHNP will maintain and keep current all credentials and continuing education as required by local, state and federal authorities. Furthermore, the nurse practitioner will independently stay informed of evidence-based practices in the field of addiction psychiatry medicine.

Functions as an Active Member of the Program Clinical Team

Attends and participates in all staff meetings, including medical team and clinical team meetings, as assigned

Attends all supervision meetings as assigned

Participates in quality improvement meetings and initiatives

Maintains and strengthens good working relationships with members of the clinical teams

Communicates and coordinates with staff from other departments effectively.

Maintains cleanliness and safety of work environment

Performs related tasks as assigned

Develops policies and procedures as needed

Qualifications

EDUCATION AND EXPERIENCE REQUIRED

Master level nursing degree.

Current licensing/certification as a Psychiatric Mental Health Nurse Practitioner.

Current BLS CPR & First Aid Certification

Management Experience is a plus, but not required PHYSICAL DEMANDS
