Your Role:

As a Case Manager, you'll play a vital role in implementing, monitoring, and assessing the housing and social service plans for families enrolled in various programs. Your responsibilities will include:

Conducting family and individual intake and maintaining accurate case records.

Collaborating with new residents to design individualized housing, mental, and medical treatment plans.

Advocating for families and individuals in securing entitlements on their behalf.

Assisting clients in obtaining permanent housing and making referrals to special housing agencies.

Meeting with clients to develop strategies to meet their housing needs.

Coordinating permanent housing searches and accompanying families to view apartments.

Referring clients to additional services such as mental health and medical services.

Liaising with daycare and schools to monitor the admission and attendance of clients' children.

Following up on child welfare cases, court cases, and parole/police matters.

Collaborating with Shift Supervisors and Resident Aides to support families effectively.

Participating in weekly Interdisciplinary Team and Administrative meetings.

Completing and entering client case management records in the DHS-CARES system.

Requirements:

High School Diploma required, Bachelors Degree preferred.

Experience in social services, housing support, mental health services or a related field preferred.

Strong organizational and communication skills.

The ability to work effectively in a collaborative team environment.

Proficiency in maintaining accurate and timely records.

Knowledge of housing resources and entitlement programs is a plus.
