We are looking for a skills worker that can assist clients in their home and in the community with learning how to live a more independent and healthier lifestyle. A Skills worker will provide adult rehabilitative mental health services that improve psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills when these abilities are impaired by the symptoms of mental illness. The service areas would encompass: interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, medication education and monitoring, mental illness symptom management skills, household management skills, employment-related skills, and transition to community living services and housing stabilization. Needs of each client vary and are person centered.
Schedule
The schedule required for this position is Full-Time with some flexibility to include being available 1 later day in the week for early evening appointments. It will be determined between the Mental Health Practitioner and his/her clients. Four to six hours a month are required for monthly supervision and professional meetings to maintain standards of care.
Primary Responsibilities
Full-time practitioner bills an average of 35+ hours per week.
Provide Adult Rehabilitative Mental Health Services meeting all DHS and Medicaid requirements.
Teach skills to improve mental health
Develop a recovery goal, rehab goal, and objective with client
Ensure confidentiality of all records and interactions
Coordinate services with county case managers, family/guardian of client, work personnel, medical personnel, etc.
Participate in professional meetings and workshops
Develop therapeutic relationship with team and client
Provide clinical interventions under the supervision of a mental health professional.
Attend supervision meetings and participate in team-based decisions regarding all clients and services.
Clinical Supervision hours will be provided as a benefit of this role
Must be able to work schedule as outlined
Documentation
Maintain client files, update client files, update treatment plans a minimum of once every six months, and do all required documentation as specified in DHS guidelines
Complete accurate case notes in session for each billable unit using the Procentive Electronic Medical Records system
Provide Community-based Services
Must have current driver’s license in good standing
Have comprehensive auto insurance
Maintain own reliable transportation
Provide services in various billable locations
Maintain staff and professional relationships with clients
Public Relations/Networking/Liaison
Develop and maintain positive working relationships within the community
Refer clients to other professionals as needed with supervisor approval
Communicate with other providers involved
Practitioner will educate self on community resources
Have knowledge of all agency programs
Qualifications
Bilingual English/Somali
Associates or bachelor’s degree in one of the behavioral sciences from an accredited college or university - or up to 32 hours of psychology related classes. Transcripts will be required to verify qualifications.
Minimum of 2000 hours of verifiable work experience providing services and support to adult individuals with Mental Illness, Traumatic brain injury or developmental disabilities and substance use disorders.
Or, Master’s degree in one of the behavioral sciences from an accredited college or university
Valid driver’s license and car insurance
