Job Description
Salary:
The Case Manager works with rehabilitation patients with various disabilities including, but not limited to: spinal cord injury, brain injury, cerebrovascular accident, neurological disorders, orthopedic conditions, etc. Monitors and manages clinical and financial coordination of treatment plan of assigned patients to ensure timely, cost-effective, individualized service delivery.
Responsibilities
Communicate effectively with doctors, patients, health care entities to gather information, provide updates, and coordinate patient care
Ensure all necessary documentation and paperwork is completed in a timely and accurate manner
Assist in coordinating patient care and follow-up appointments.
Provide support and guidance to patients and their families throughout the case management process
Understands, complies with and makes individual centered decisions based on all required areas of specific programs that apply to case management services
Requirements
Valid Driver's License with clean driving record
Two year's direct service experience with persons with disabilities
Experience in completing assessments, service plan development and monitoring
Effective organizational and communication skills
