Join a purpose-driven community of champions.Position Purpose: To implement performance improvement functions in accordance with written agency policies and procedures and accreditation standards. To support technical development and implementation of the software that supports the Electronic Service Record.Qualifications/Basic Job Requirements:

High School diploma or GED (Bachelor’s preferred)

Demonstrated ability in Microsoft Office

Excellent written communication skills

Excellent verbal communication skills

Excellent critical thinking skills

Knowledge of basic statistics

Must be able to maintain confidentiality of highly sensitive informationEssential Responsibilities:1. Evaluate and convert raw data into reliable information and prepare reports in a usable format based on an understanding of administrative needs.2. Help in determining data needs and report functionality based on end user needs.3. Coordinate and facilitate Performance Improvement Committees- including scheduling, compiling and distributing minutes and other documentation, sending reminders of activities due as well as any needed follow up and serving as the implementation champion and change leader.4. Participate in Performance Improvement Teams, Ad Hocs, Root Cause Analyses and Intense Analyses as directed. Duties often include: scheduling, taking minutes, reminding staff of upcoming tasks. 5. Manage and update the electronic survey system as needed, creating new surveys for a variety of audiences and various departments.6. Maintaining all aspects of the electronic Internal Notification system platform. Including assigning user roles, training, and ensure follow up is completed. 7. Compile Internal Notification Form data from the electronic platform and prepare graphic and written reports for presentations.8. Manage and update the Policy and Procedure software system (Policy Tech) including loading all new staff into the system, providing training and assistance to staff in operating Policy Tech software, and assigning Policies and Procedures as required.9. Develop and create Standard Operating Procedures related to performance of job-specific duties.10. Meet all required due dates.11. Create data-gathering tools (forms/spreadsheets) as needed.12. Compile data through the use of spreadsheets / databases / ESR queries and convert into information for reports.13. Perform accurate statistical calculations.14. Understand audience needs to determine the most effective mode of data display. 15. Prepare graphic displays of data, e.g. control charts, bar charts, graphs, etc. 16. Serve as resource and provide training to staff, as necessary, in PI related topics.17. Compile data and convert to information for scorecards and dashboards.18. Help maintain and verify data for all reports and dashboards. Assist in new report and dashboard development.19. Utilize critical and logical thinking skills to evaluate validity, accuracy and reliability of reports.20. Complete audits as needed to ensure compliance with all rules, regulations and accreditation requirements.21. Maintain organized electronic and hard-copy files of raw data and PI reports.22. Participate in tracers and other accreditation activities as necessary.23. Serve as a member of the Performance Improvement Department team and participate in all team meetings and activities.24. Exercise confidentiality in keeping with the Code of Ethics and with the framework of the law.25. Deliver exceptional customer service consistently to every customer.26. Serve as a role model for other staff, patients and customers and demonstrate positive guest relations in representing Rosecrance.

Schedule: 8-hour shifts

Shift: 8:30am - 5:00pm

Monday through Friday

Work Location: Rosecrance Rockford East - Rockford, IL
