Licensed Vocational Nurse (LVN)

Job Description

Stonebrook Healthcare Center is looking for a Licensed Vocational Nurse (LVN) to join our amazing team! The right candidate will have a current LVN license with preferred experience in a skilled nursing facility. We are looking for someone with excellent interpersonal skills and has the ability to work a PM shift or NOC shift schedule.

Responsibilities

Under the direction of the DON

Responsible for overall care of residents

Provide care utilizing infection control measures that protect both the staff and patient

Assure continuity of quality patient care delivered with appropriate documentation

Prepare regular clinical/progress notes and summaries

Maintain resident confidentiality

Qualifications

Proof of current LVN license

Proof of current CPR certification

Ability to work PM shift schedule

Experience as an LVN in a clinical setting preferred - may negotiate wage with experience

Excellent interpersonal skills

Valid identification and reliable transportation
