The Support Broker will support members enrolled in Agency Brokerage Department to lead a fulfilling Self-Directed life. Agency Support Brokers work with Self-Directed Participants/Designees to create a Budget and Habilitation Plans that are person centered.

Minimum Qualifications: The position requires 1 year experience working with people with Intellectual Disabilities and OPWDD required initial Broker trainings.

Assist in developing a comprehensive Self-Directed Budget that is consistent with the Individual Service Plan (ISP).

Assist with amending any changes to the Self-Direction Budget and plan, if needed.

Ensure the Self-Directed Budget and Plan is person centered.

Provide education and guidance to the entire Circle of Support Team on the regulations set forth by Office for People with Developmental Disabilities and New York State.

Attend all Individualized Service Plan meetings, and any other meetings SD Participant request. Assist with Monthly paperwork, which can include but not limited to, Expense Report, Mileage Reimbursement, Self-Hired Staff Timesheets, etc.
