Position Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide professional nursing care to patients in healthcare and community settings. The LPN will work collaboratively with physicians, Registered Nurses (RNs), nurse practitioners, healthcare aides, and other healthcare professionals to deliver high-quality patient care.

LPNs may provide care in hospitals, nursing homes, rehabilitation centres, clinics, community healthcare programs, and private home settings.

Key Responsibilities

Provide nursing care based on individual patient needs and established care plans.

Monitor, assess, and document patients’ health conditions and report changes appropriately.

Administer medications, injections, and prescribed treatments according to professional standards.

Perform wound care, dressing changes, catheter care, and other clinical procedures.

Assist with chronic disease management, rehabilitation programs, and ongoing patient support.

Support patients with mobility, personal care, and activities of daily living.

Educate patients and families regarding medications, treatments, health management, and preventative care.

Maintain accurate patient records and electronic health documentation.

Collaborate with physicians, Registered Nurses, healthcare aides, therapists, and caregivers.

Follow infection prevention practices, workplace safety guidelines, privacy requirements, and professional nursing standards.

Provide compassionate care to seniors, individuals with disabilities, and patients requiring ongoing healthcare support.

Qualifications

Required

Current Licensed Practical Nurse (LPN) license in the applicable Canadian province.

Completion of an approved Practical Nursing program.

Current CPR/BLS certification.

Strong communication, organization, and clinical assessment skills.

Ability to work independently and effectively within a healthcare team.

Commitment to providing safe, respectful, and patient-centered care.

Preferred

1–3 years of practical nursing experience.

Experience in one or more of the following areas:

Long-term care

Home healthcare

Community nursing

Rehabilitation

Acute care

Chronic disease management

Experience using electronic medical records (EMR) and healthcare documentation systems.
