Primary Care Nurse Practitioner / Physician Assistant – Adult & Geriatric Care West Bend, Beaver Dam, Fond du Lac

Full-Time Monday–Friday 7:00 AM – 5:00 PM

No Nights • No Weekends • No Holidays

Oak Medical SC is seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to provide acute and chronic care management for adult and geriatric patients throughout our West Bend, Beaver Dam and Fond du Lac service areas.

In this role, you'll collaborate with physicians, specialists, and facility care teams to develop and implement comprehensive treatment plans that improve patient outcomes and quality of life.

What You'll Do

Provide primary care services for adult and geriatric patients with acute and chronic medical conditions.

Evaluate patients, establish diagnoses, prescribe medications, and perform diagnostic and therapeutic procedures within scope of practice.

Develop individualized treatment plans in collaboration with physicians and interdisciplinary care teams.

Coordinate care with specialty providers, including wound care, psychiatry, and other consulting services.

Communicate changes in patient condition and treatment plans with patients, families, facility staff, and healthcare team members.

Maintain timely, accurate, and compliant documentation.

Build strong relationships with patients, families, and facility staff to support exceptional care.

Perform other duties as assigned.

Required Qualifications

Current Nurse Practitioner or Physician Assistant license.

Active DEA registration.

BLS certification.

Strong clinical assessment and diagnostic skills.

Ability to work independently while collaborating effectively with interdisciplinary teams.

Commitment to compassionate, patient-centered care.

Preferred Qualifications

Leadership experience or demonstrated leadership skills.

Strong organizational and time-management abilities.

Positive, solution-oriented attitude.

Experience working with adult and geriatric populations.

Work Setting

Onsite patient care within assisted living, skilled nursing facilities, specialty hospitals, and senior living communities
