Purpose of the Role

To provide high-quality, person-centred care and support to individuals in their own homes and supported living settings, enabling them to maintain independence, dignity, choice and wellbeing while ensuring their health, safety and welfare.

Key Duties and Responsibilities

Provide personal care in accordance with individual care plans, including assistance with washing, bathing, dressing, grooming, continence care, mobility and eating where required.

Support individuals with medication prompts and assistance in accordance with company policies and care plans.

Assist service users with moving and positioning using appropriate equipment and safe moving and handling techniques.

Prepare meals and support individuals with nutrition and hydration requirements.

Undertake domestic tasks including cleaning, laundry, changing bedding and maintaining a safe and hygienic living environment.

Accompany and support service users to attend medical appointments, community activities, shopping and social engagements.

Promote independence, dignity, privacy, choice and inclusion, encouraging individuals to participate in daily living activities wherever possible.

Observe and monitor the physical and emotional wellbeing of service users and promptly report any changes or safeguarding concerns to the Registered Manager or Care Coordinator.

Respond appropriately in emergency situations and follow company emergency procedures.

Maintain accurate daily care records, medication records and incident reports using company recording systems.

Work collaboratively with colleagues and external healthcare professionals, including GPs, district nurses, occupational therapists and social workers, to ensure continuity of care.

Follow all Care Quality Commission (CQC) standards, safeguarding procedures, health and safety legislation, infection prevention and control procedures and company policies.

Attend mandatory training, supervision sessions and team meetings to maintain professional competence.

Skills and Experience

Essential

Compassionate, caring and professional approach.

Good communication and interpersonal skills.

Ability to work independently and as part of a team.

Understanding of safeguarding, confidentiality and person-centred care.

Willingness to undertake mandatory training.

Desirable

NVQ Level 2 or Level 3 Diploma in Health and Social Care (or equivalent).

Previous experience working in domiciliary care, supported living or residential care settings.

Full UK driving licence (where required).

Hours of Work

37.5 hours per week on a rota basis, including evenings, weekends and public holidays as required by service delivery.
