Implement and manage personalized patient care plans.

Provide mobile support to the Gillespie, IL community.

Maintain accurate and timely clinical documentation.

Efficiently manage your travel time and daily schedule.

Schedule: Monday to Friday, 8:00 AM – 5:00 PM. No on-call or weekend requirements!QualificationsCore Requirements

Associate degree or higher in Physical Therapy.

1+ years of professional experience in Home Health Care.

Proficiency with Electronic Medical Record (EMR) systems (e.g., Practice Partner).

Valid Driver's License and a Physical Therapy Assistant license in Illinois.

Full-time
