Field Case Manager - Behavioral Health

Job Description

A-Line Staffing is now hiring a Field Case Manager – Behavioral Health. This will be full time.

Work Setting/Location: Must live in one of the target counties listed below due to required field travel. This is a hybrid field role with work-from-home time between member visits.

(MUST LIVE IN ONE):

McLean, De Witt, Macon, Champaign, Douglas, Shelby, and surrounding counties

Schedule: Monday–Friday 8:00 AM – 5:00 PM CST

Job Type: Project Role (May extend beyond 90 days based on need and performance)

Pay Rate: $27.70 an hour + mileage reimbursement

Benefits: medical, dental, vision, life insurance active after 90 days

Responsibilities

Conduct comprehensive member assessments using care management tools and data review

Evaluate eligibility, needs, risks, and available benefits/resources

Identify high-risk factors impacting member health outcomes and care planning

Develop, implement, and monitor individualized care plans

Coordinate care services with providers, community resources, and support systems

Participate in case conferences with clinical teams, supervisors, and medical directors

Identify and escalate quality of care concerns appropriately

Use motivational interviewing and coaching techniques to support behavior change and engagement

Support members in making informed healthcare decisions

Document all case management activities in compliance with regulatory and organizational standards

Ensure adherence to case management and quality management processes

Required Qualifications

Bachelor’s degree in behavioral health or human services REQUIRED

(Psychology, Social Work, Marriage & Family Therapy, Nursing, Counseling, or related field)

Minimum of 1 year of experience in: Behavioral health, Social services or related human services field preferred

Experience working with elderly and/or disabled populations

Experience working with Medicaid/Medicare populations
