About the Role
We are seeking a compassionate and resourceful Case Manager to join the Behavioral Health Department at Cornell Scott Hill Health Center. In this role, you will coordinate care and connect clients with community resources and support services to promote stability, wellness, and successful treatment outcomes. The Case Manager will advocate on behalf of clients, assist with access to entitlements and basic needs, support engagement in treatment services, and participate in discharge planning. This position is ideal for someone who is passionate about behavioral health, community-based care, and supporting diverse populations through a collaborative and person-centered approach.
Key Responsibilities
Coordinates referrals with other Providers in a timely and professional manner
Follows-up with referral sources in a timely fashion as required
Promote coordination of service delivery and prevent duplication of services
Follows-up on results of services provided to clients in a timely fashion
Actively advocates on behalf of the clients with community resources
Facilitates psycho-educational groups as assigned
Always maintain confidentiality and ensure to follow agency, state and HIPPA laws
Attends Provider's meetings in the community to foster collaborative relationships
Responsible for participating in discharge planning as needed including housing
Completes applications for Basic Needs, transportation, housing and treatment programs in a timely fashion
Coordinates with community agencies for outreach/engagement. Establishing and maintaining good working relationships with community agencies
Attends and participates in team meetings
Performs all other duties as assigned by the Program Director Required Qualifications
High School Diploma/GED required. Associate's Degree, Bachelor's Degree and /or CADAC preferred.
Human Service experience is preferred.
Excellent working knowledge of New Haven community social and health service agencies is preferred.
Excellent organizational skills are required to maintain the caseload.
Ability to generate high level of client satisfaction.
Knowledge and skills related to individual, group and family system dynamics.
Knowledge of and demonstration of professional ethics is essential.
Ability to work well with multi-disciplinary service providers.
Must demonstrate age specific/population specific competencies in area(s) of practice.
Knowledge and skills related to individual, group and family system dynamics.
Must demonstrate cultural competence.
