Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

MINIMUM REQUIRED QUALIFICATIONS – MA RN license; BLS; 2+ years minimum of acute care hospital experience PREFERRED QUALIFICATIONS – 2-3 years case management experience LENGTH OF ASSIGNMENT – 13 weeks SHIFT / HOURS PER WEEK – M-F, 40 hours 8a-4:30p.

Weekends on request.
