Join Us as a Clinical Pharmacist (Home Infusion & Specialty Pharmacy)!
Vital Care of Largo is looking for a Clinical Pharmacist who brings both clinical judgment and hands-on dispensing experience to a growing home infusion pharmacy. In this role, you’ll support complex patients outside the hospital setting, ensuring safe, compliant sterile compounding and meaningful clinical oversight. Your work will directly impact patient outcomes by enabling high-quality infusion therapy in the comfort of patients’ homes.
Your Responsibilities
As a Clinical Pharmacist, you will:
Oversee sterile IV compounding operations and medication dispensing to ensure accuracy, safety, and regulatory compliance
Review and verify prescriber orders, clinical documentation, and laboratory results to support appropriate therapy
Provide patient counseling, clinical assessments, and ongoing medication management for home infusion and specialty pharmacy patients
Supervise and support trained sterile compounding technicians, reinforcing best practices and compliance standards
Ensure adherence to USP <797>, accreditation requirements, and applicable state and federal regulations
Collaborate with physicians, nurses, and referral partners to coordinate high-quality, patient-centered care
Deliver therapeutic recommendations based on clinical data, patient response, and evidence-based practice
Participate in quality assurance and performance improvement initiatives as part of the pharmacy team
Your Qualifications
To excel in this role, you need:
Required:
Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy (BS Pharm)
Active RPh license in good standing with the Florida Board of Pharmacy
2+ years of clinical pharmacy experience
Ability to interpret laboratory results and manage complex medication regimens
Strong communication skills with patients, providers, and internal care teams
Demonstrated clinical judgment with a high level of accuracy and attention to detail
Preferred:
PGY1 residency or equivalent clinical experience
Experience in home infusion therapy and sterile compounding
Formal sterile compounding training
Experience supervising pharmacy technicians or support staff
Board certification (e.g., BCSP, BCNSP, BCPS, or related specialty)
Multistate licensure or willingness to reciprocate licensure as needed
Spanish or additional language proficiency
