Nurse Practitioner / Physician Assistant – Part-Time Skilled Nursing Facility (SNF) Wound Care Orange County We are seeking an experienced and independent Nurse Practitioner or Physician Assistant to provide advanced wound care services within Skilled Nursing Facilities (SNFs) throughout the Orange County area and surrounding communities.

As a specialty wound care practice, we focus on delivering evidence-based treatment that improves healing outcomes and reduces avoidable hospitalizations.

This facility-based, part-time role is ideal for providers who value autonomy, flexible scheduling, and the opportunity to deliver high-impact care within interdisciplinary care teams.

Minimum availability of 3-4 days per week required.

Position Overview Part-time Facility-based patient care within SNFs Independent, on-site clinical practice Travel within assigned Fresno territory Flexible scheduling based on availability Clinical Responsibilities Perform comprehensive wound assessments for SNF residents Develop and manage individualized, evidence-based treatment plans Perform advanced wound care procedures (e.g., debridement, dressing selection) Monitor healing progress and adjust care plans as needed Collaborate with Directors of Nursing (DONs), nursing staff, and interdisciplinary teams Participate in care planning and IDT meetings as appropriate Educate facility staff on wound care best practices and prevention strategies Maintain accurate, timely, and compliant EMR documentation Support regulatory compliance and quality measures related to wound care Identify at-risk residents and implement preventative interventions Qualifications Required: Active, unrestricted California NP or PA license Current DEA registration Current BLS certification Minimum 1 year of clinical experience Reliable transportation and ability to travel locally Preferred: Wound care experience Experience with lower extremity or vascular assessment Proficiency with EMR documentation systems Flexible with scheduling and travel within assigned service areas Ability to reposition or turn patients as needed without guaranteed assistance Bilingual (English/Spanish) Service Area – Orange County This position primarily serves Skilled Nursing Facilities throughout Orange County and surrounding communities, including: Irvine, Anaheim, Santa Ana, Orange, Tustin, Costa Mesa, Newport Beach, Huntington Beach, Fountain Valley, Garden Grove, Westminster, Fullerton, Yorba Linda, Mission Viejo, Laguna Hills, Lake Forest, San Clemente, and surrounding areas.

Expanded coverage opportunities may be available for providers open to additional areas throughout Los Angeles County, the Inland Empire, and nearby Southern California communities.

Territory assignments are structured to promote efficient routing and minimize unnecessary travel whenever possible.

Compensation Part Time W-2 Structure Earning potential varies based on daily visit volume and provider availability.
