Rate: 40.3 Requirements:

Active RN license in practicing state required.

Experience in SNF, LTC, or rehab preferred.

Current BLS required; ACLS may be required.

Strong assessment and critical-thinking skills.

Knowledge of SNF regulations and infection control.

Ability to provide compassionate, patient-centered care. Responsibilities:

Patient Care & Assessment:

Assess and monitor patient conditions regularly.

Administer medications, IV therapy, and treatments.

Develop and implement individualized care plans.

Documentation & Compliance:

Document all patient care and progress in the EMR.

Ensure patient safety and compliance with policies.

Collaborate with interdisciplinary team members.

Education:

Educate patients and families on care plans and health.
