Job Description
The Registered Nurse Case Manager plays a vital role in coordinating and managing care for patients primarily in acute care settings. This position involves working within a medium-sized team of 6-10 members to ensure optimal care planning, effective case coordination, and safe discharge processes. The role requires occasional local travel and proficiency with Electronic Health Records (EHR) to document and track patient progress accurately.
8 week contract with the option to extend
Shifts: 4x10-Hour (07:30 - 18:00)
Weekends Required: Yes - every other weekend
Years of Experience: 1 year(s) - minimum
Must have Acute Care Experience, Discharge Planning/Care Coordination Experience
Charting: Epic - Required
Certs Required: BLS, RN Licensure

Develop and implement individualized care plans for acute care patients
Coordinate case management activities within a medium-sized team

Collaborate with healthcare professionals to optimize patient outcomes
Conduct occasional local travel to support patient needs and care coordination
