Position Overview
Our healthcare staffing agency is actively seeking an experienced MDS Coordinator (RN or LPN/LVN) for a 13 week contract assignment at a reputable facility in Canton, South Dakota. This role is ideal for a nurse with strong assessment skills, deep knowledge of the RAI process, and the ability to manage MDS timelines with precision.
Key Responsibilities
MDS Coordination — Complete OBRA and PPS assessments accurately and on schedule
Care Plan Leadership — Develop and update individualized care plans based on resident needs
Interdisciplinary Team Collaboration — Partner with nursing, therapy, social services, and dietary teams
PDPM Documentation — Ensure accurate coding and documentation to support PDPM reimbursement
Regulatory Compliance — Maintain compliance with CMS and state guidelines
Quality Measures Monitoring — Track and support improvement of facility QMs
Resident & Family Engagement — Participate in care conferences and communicate assessment outcomes
Required Qualifications
Active RN or LPN/LVN license (South Dakota)
Minimum 2 recent years of MDS experience in a nursing facility -- LTC/SNF
Strong working knowledge of RAI, MDS 3.0, and PDPM
AHA BLS certification (current and active)
Excellent assessment, documentation, and time management skills
Ability to work independently and meet strict assessment deadlines
