Pay: $24/hour ($25/hour for bilingual Spanish-speaking candidates)

XstremeMD is seeking a detail-oriented and clinically strong LPN Case Manager to join our growing team. This is a remote, telephonic role focused on coordinating care, supporting patients, and improving health outcomes — all without direct patient contact.
A top-performing nurse in this role combines clinical judgment, strong communication skills, and the ability to work independently in a fast-paced environment.
Key Responsibilities
Conduct telephonic case management and patient follow-up
Perform clinical assessments and identify patient needs
Document all interactions accurately in EMR systems
Collaborate with internal clinical teams to support care plans
Ensure timely and appropriate case resolution
Maintain high standards of customer service and patient satisfaction
Manage caseload efficiently while meeting productivity expectationsRequirements:
Minimum Qualifications
Active LPN compact license
Strong critical care assessment skills
Proficiency with:
Electronic Medical Records (EMR)
Microsoft Office Suite (Word, Excel, Outlook)
Strong computer literacy and ability to navigate multiple systems
Excellent communication and critical thinking skills Preferred Experience
Emergency Room (ER) or ICU experience
Correctional/prison nursing experience
