XstremeMD is seeking an experienced and highly skilled Registered Nurse Case Manager to provide telephonic case management and clinical support. This role is part of a team that delivers 24/7 medical coverage, requiring flexibility in scheduling while striving to maintain strong work-life balance.
This position is ideal for nurses who excel in critical thinking, independent decision-making, and high-level clinical assessment — all without direct patient contact.Key Responsibilities
Perform telephonic triage, assessment, and case management
Coordinate and monitor patient care plans
Serve as liaison between patients, providers, employers, and carriers
Ensure timely documentation in EMR systems
Support high-quality, cost-effective care and return-to-work outcomes
Maintain consistent communication and follow-up on assigned cases
Meet productivity and quality benchmarksRequirements:
Minimum Qualifications
Active Registered Nurse (RN) license (multi-state/compact preferred)
Minimum 2+ years of clinical nursing experience
Strong critical care assessment skills
Proficiency in:
EMR systems
Microsoft Office Suite
Strong computer literacy and ability to multitask across systems
Excellent communication, organization, and critical thinking skills
Preferred Experience
Emergency Room (ER) or ICU experience
Case management or workers’ compensation experience
