This group is seeking a full-time Travel Nurse Practitioner to provide palliative care to patients in their homes. You will be assigned a geographic territory where you will provide excellent care to a small number of patients per day. This position will give you the opportunity to be involved in creating a program that will bring healthcare to those who are in real need and can help change the course of their lives.

Job Responsibilities:

*You will be traveling in a designated territory providing care to a small number of patients daily.

*Develop and implement clinical plans of care for adult patients facing chronic and complex conditions

*History and physician examination of the patient

*Educate patients and family members on chronic health conditions, advance directives, disease progression and end of life symptom management

*Identify gaps in care and unmet needs of patients and initiate appropriate referrals

*Collaborate and participate with an interdisciplinary team in development and review of comprehensive care plans.

*Prescribe medication as permitted by the state board of Nursing

Group is seeking a Nurse Practitioner who is engaged, enthusiastic and open to learning. This candidate must enjoy working with geriatric patients. Group will consider a new graduate with the above mentioned attributes. They are willing to train a new grad who wants to learn and understands that healthcare is fast moving and ever changing.

Job Requirements

Family or Adult Nurse Practitioner with appropriate certifications and licensure required

Previous experience in home-based services, complex cases and/or palliative care is preferred, some bedside hospital, oncology, ER or ICU experience a pus

Must have reliable transportation and valid driver's license

Possession of DEA

CPR certification
