Position: Licensed Practical Nurse (LPN) – Skilled Nursing Facility

Location: Warren, OH

Job Description:

Provide direct nursing care to residents in a skilled nursing facility

Administer medications and treatments as ordered

Monitor vital signs and document patient status

Collaborate with RNs, physicians, and care team

Assist with care planning and patient education

Requirements:

Active Ohio LPN license

2+ years of nursing or long-term care experience preferred
