We have an exciting opportunity for a full-time Director of Clinical Community Based Services to join our leadership team in Albany, New York. The team will oversee Albany, Broome, Columbia, Fulton, Greene, Montgomery, Schenectady, Saratoga, Rensselaer, Tioga, Washington and Schoharie counties.

The MSN Director will provide clinical leadership and direct supervision for community based care managers and clinical manager as they provide care coordination services to CDPHP HARP members programs. This includes the development and operations of these clinical programs. Develops community based methodology to engage members in managing their chronic physical health or behavioral health conditions. Collaborates with clinical leaders for all lines of business and contracted vendors to implement purchased programs. Conducts oversight of contracted vendors who provide services in the home to ensure contractual commitments and goals are met. Oversees the daily operations of the community based programs to ensure that they are efficient and structured to meet the program goal and improve care. Maintains direct accountability for reporting, compliance, financial analysis and planning and coordination of efforts with other departments to assure effective operations of the community based programs. The staff will work with our partners at Capital District Physicians' Health Plan (CDPHP). This position will require around 75% travel.


• Adjusts interventions based on review of outcomes.

• Collaborates and partners within CDPHP participating providers

• Collaborates with the physical and behavioral health providers to develop community based programs that support the providers plan of care for their patients. Develops communication strategies that reinforce provider collaboration for clinical programs.

• Community Care, and regional health homes to develop effective community based programs that address the quality of care, access of services and health care disparities

• Coordinates communications between CDPHP, providers, or vendors who are providing community based services and Community Care.

• Develops innovative community based engagement strategies for members with complex needs, those who are not well connected with a provider, and to reduce admissions and emergency room use.

• Ensures community based services meet applicable regulatory, government agency, and accrediting compliance requirements.

• Ensures the attainment of strategic business objectives by identifying, recruiting, developing, directing, and evaluating staff.

• Informs and advises Regional Director and other Community Care executive leadership of current trends, issues, problems and activities, and develops and implements appropriate interventions to facilitate problem resolution and policy making.

• Perform duties and responsibilities in accordance with the philosophy and standards of Community Care Behavioral Health, including conveying courtesy, respect, enthusiasm and a positive attitude through all contacts with staff, health plan members, providers, peers and visitors.

• Reviews performance of community based services, including the performance of contracted providers, health homes or vendors. Analyzes utilization and cost data. Based on analysis, designs and implements focused community based interventions to impact identified trends. Establishes and reviews outcomes metrics to evaluate impact of interventions in altering negative trends and meeting goals.

• Reviews staff performance on an ongoing basis to ensure protocols and methods are used.


• Currently licensed MSN required; Licensed Nurse practitioner (NP) preferred.

• 5-7 years of health / behavioral health care experience required.

• 5-7 years of managed care experience preferred.

• Excellent communication skills; verbal, written and interpersonal.

• Above average analytical and problem solving skills.

• Strong leadership skills and independent decision making ability.

• Computer skills in Word, Excel and Outlook required.

• Supervisory experience required.

Job Insight:
