Position Summary

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Skilled Nursing Facility team located in Plattsburg, MO. The LPN is responsible for providing direct nursing care to residents, ensuring high-quality clinical outcomes, and supporting a safe, respectful, and person-centered environment.

Essential Duties and Responsibilities

Provide direct nursing care to residents in accordance with care plans, physician orders, and facility policies

Administer medications and treatments accurately and timely

Monitor and document resident conditions, reporting changes to the RN and physician as appropriate

Assist in the development, implementation, and updating of individualized care plans

Supervise and delegate tasks to Certified Nursing Assistants (CNAs)

Ensure compliance with all federal, state, and local regulations (CMS guidelines)

Maintain accurate and timely documentation in the electronic medical record (EMR)

Respond to resident needs, emergencies, and call lights promptly

Promote infection control, safety, and quality assurance practices

Communicate effectively with residents, families, and interdisciplinary team members

Qualifications

Current, active Licensed Practical Nurse (LPN) license in the state of [State]

Current CPR/BLS certification

Previous experience in long-term care or skilled nursing preferred

Strong clinical, organizational, and communication skills

Ability to work collaboratively in a team environment

Knowledge of geriatric nursing and regulatory requirements preferred
