Position Summary
The Physician provides comprehensive, patient-centered medical care to individuals and families served by Crossroads Family Care. This role is responsible for assessment, diagnosis, treatment, preventive care, and care coordination across the lifespan. As part of the multidisciplinary care team, the Physician supports quality outcomes, continuity of care, and integrated services while promoting compassionate, evidence-based care.
Essential Duties & Responsibilities
Clinical Care & Patient Services
Provide comprehensive primary care services to patients of all ages.
Evaluate, diagnose, and treat acute and chronic medical conditions.
Order, review, and interpret laboratory tests, imaging, and diagnostic studies.
Prescribe medications and treatment plans in accordance with clinical standards.
Perform preventive care, wellness exams, and chronic disease management.
Refer patients to specialists or community resources as appropriate.
Patient Education & Care Coordination
Educate patients and families regarding diagnoses, treatment plans, medications, and preventive health.
Coordinate care with specialists, hospitals, and internal care teams.
Promote continuity of care and patient engagement.
Support whole-person care through collaboration with behavioral health and dental teams.
Documentation & Compliance
Maintain accurate, timely, and complete documentation in the EHR.
Ensure compliance with HIPAA, HRSA, OSHA, and applicable regulatory standards.
Follow organizational policies related to quality, safety, and risk management.
Participate in audits, chart reviews, and quality initiatives.
Quality & Team Collaboration
Participate in quality improvement and performance initiatives.
Attend meetings, trainings, and provider collaboration activities.
Support efficient patient flow and positive clinic operations.
Maintain professional and ethical standards at all times.
Qualifications
Required
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution.
Current, unrestricted license to practice medicine in the State of Texas.
Current DEA registration.
Board eligible or board certified in Family Medicine, Internal Medicine, Pediatrics, or related specialty.
Current BLS certification.
Preferred
Experience in an FQHC, outpatient, or community health setting.
Experience with EHR systems.
Knowledge, Skills & Abilities
Strong clinical assessment and diagnostic skills.
Knowledge of evidence-based primary care practices.
Strong communication and patient education skills.
Ability to manage a patient panel and prioritize effectively.
Commitment to compassionate, culturally responsive care.
Work Environment & Physical Requirements
Clinical setting with frequent patient interaction.
Requires standing, walking, sitting, and assisting patients for extended periods.
Ability to respond to urgent clinical situations.
Occasional lifting up to 25 pounds.
