Job Title: Nurse Practitioner (Traveling – Personal Care Homes)

Location: Buford & Gainesville, GA

Schedule: Wednesdays & Thursdays (Preferred)

Compensation: Per Chart Note (1099/Contract)

EAN Wellness Services is a growing, family-owned healthcare company dedicated to providing compassionate, high-quality care to our community. We are currently seeking a dependable and motivated Nurse Practitioner to provide on-site care in personal care homes located in Buford and Gainesville, GA.

This role focuses primarily on chronic care management, working closely with patients to monitor, manage, and improve ongoing health conditions in a comfortable, residential setting.

Responsibilities:

Provide comprehensive patient assessments in personal care home settings

Manage and monitor chronic conditions (e.g., hypertension, diabetes, etc.)

Complete and submit accurate chart notes for each patient encounter

Collaborate with facility staff to ensure continuity of care

Maintain compliance with all clinical and documentation standards

Qualifications:

Active Nurse Practitioner license in the state of Georgia

Strong clinical assessment and documentation skills

Experience in primary care, geriatrics, or chronic care management preferred

Ability to work independently and travel between locations

Reliable transportation required

Compensation & Schedule:

Paid per completed chart note
