**Description:**
**Palliative Care Intake Coordinator**
**Position Purpose:**
The Palliative Intake Coordinator collaborates with hospital discharge planners/social workers, liaison personnel, physicians, billing specialists, patients, families, and others to receive information which facilitates admission of Palliative patients; documents appropriate demographic and clinical information for the processing of incoming referrals; coordinates communication process between referral sources, third party payors, and professional/clerical staff; and coordinates referral process from initiation to completion including all follow-up necessary to accomplish expected outcome.
**Minimum Qualifications:**
+ Education: High School diploma/GED
+ Experience: Minimum of one year recent Hospice experience -- or related setting such as home health, hospital, ambulatory care center, physician office in which assessment and critical decision-making skills are required
+ Effective Communication Skills
+ Demonstrated ability regarding critical thinking skills, telephone and direct assessment of patient, responding to emergency referrals, establishing priorities re: scheduling admissions of Palliative patients in all settings-home, hospital, nursing home, and assisted living center.
+ Effective Interpersonal skills, demonstrates tact, judgment and diplomatic business-like manner in dealing with physicians, other healthcare professionals and coworkers
+ Intermediate to advanced Excel skills
