CCBYS Case Manager

Status: Exempt Reports To: CCBYS/Hilltop Shelter SupervisorAbout the Role
We are seeking a dedicated Case Manager to support high-risk youth and their families through crisis intervention and ongoing case management. This position works closely with law enforcement, families, and community partners to stabilize situations, reduce court involvement, and promote safe, successful outcomes for youth.Key Responsibilities
Respond to crisis calls involving youth and families, often in coordination with law enforcement
De-escalate family conflicts and help prevent court involvement or out-of-home placement
Develop and monitor individualized service plans with measurable goals
Maintain accurate case documentation, including notes, reports, and referrals
Connect clients to community resources and support services
Support youth and families through court processes, including attending hearings when needed
Coordinate placement and reunification efforts when necessaryQualifications
Bachelor's degree required
At least 21 years of age
Experience working with youth and families preferred
Knowledge of trauma-informed care and willingness to apply it in practice
Valid Illinois driver's license, reliable transportation, and proof of insurance
Ability to pass background checks, including CANTSWhat We're Looking For

Ability to travel locally and respond to calls during day or evening hours

Proficiency with computers and documentation
