metro area\n\n\nRequired Qualifications\n\n Licensed Physical Therapist (PT) in Maryland or eligible for Maryland licensure\n Minimum 1 year of experience as a Physical Therapist, ideally in geriatric care, outpatient, or nursing/long-term care settings\n Experience with OASIS (opening) preferred\n Familiarity with documentation systems such as EMS-Myunity and HCHB is a plus\n Strong clinical assessment skills and experience creating individualized rehabilitation plans\n\n\nKey Responsibilities\n\n Deliver high-quality physical therapy and rehabilitation services to residents in Assisted Living, SNF, and Long-Term Care units\n Perform patient assessments, develop and implement individualized care and treatment plans focused on mobility, strength, balance, and function\n Document patient progress and plan updates accurately using facility documentation systems (EMS-Myunity, HCHB)\n Collaborate with interdisciplinary teams (nursing, OT, SLP, social work) to coordinate care and support resident independence\n Provide education to residents, families, and staff on therapeutic activities, fall prevention, and safe mobility techniques\n Adjust treatments based on resident response and clinical goals to maximize outcomes in long-term care and assisted living populations\n\n\nWhy This Role?\n\n Work in a supportive continuing care community in Silver Spring, MD, with easy access to Washington, D.C.
