We are seeking a dependable and experienced LPN/LVN for an assignment in a nursing home setting. The ideal candidate will provide quality patient care, support the clinical team, and ensure adherence to facility standards and protocols.Key Responsibilities:

Provide direct patient care and administer medications as prescribed

Monitor patient conditions and promptly report any changes

Assist in developing and implementing patient care plans

Support CNA/PCA staff and assist with patient care duties as needed

Maintain accurate and timely documentation using PointClickCare EMR

Ensure patient safety, comfort, and well-being throughout the shift

Follow all facility policies, procedures, and infection control guidelinesRequired Qualifications:

Active LPN/LVN License (required)

2 years of recent experience in a nursing home or long-term care setting (required)

Hands-on experience with PointClickCare EMR (required)

Willingness to assist with CNA duties when necessary

Strong reliability, punctuality, and attendance recordAdditional Requirements:

No Requested Time Off (RTO) during the assignment

Candidates must demonstrate consistent attendance (excessive tardiness or absences may result in early termination)Key Competencies:

Ability to work independently during night shifts
