Flexible Schedule What does an Transitional Age Youth Program Coordinator do?
Support Program Director with recruiting, onboarding, training, scheduling, and direct supervision of program staff, interns, and volunteers
Utilize program assessments and screening tools in collaboration with youth to inform priorities of the individual service plan and make appropriate internal and external referrals for services
Oversee data entry into the Santa Barbara County Homeless Management Information System (HMIS) to track participant demographics, eligibility, and program engagement
Utilize program databases to monitor and reflect program contacts, activities, and outcomes
Ensure client records and documentation is accurate, up-to-date, and confidential
Assist with completion of various monthly, quarterly, semi-annual, and annual reporting requirements
Maintain working knowledge of local community resources and provide youth with referrals to other agencies for services when appropriate
Attend meetings with community partners to establish collaborative relationships, coordinate services, and promote continuity of care
Conduct community presentations to increase awareness and referrals to Youth and Family Services programs
Support youth to enhance their education, vocation, relationships and communication, independent living skills, physical health, mental health, and more
Assist with meeting basic needs of youth without judgment or personal opinion, building positive relationships to encourage continued progress on goals
Effectively work in a team-based care model to ensure youth receive well rounded, consistent, and coordinated support
Maintain a controlled and safe environment for youth to ensure their physical, emotional, and psychological safety, assisting with daily cleaning/sanitization and organization of workspace areas
Supervise program participants
Attend meetings as required to discuss program updates, report on cases, and identify service delivery needs
Demonstrate effective professional boundaries, self-care measures, and seek support from direct supervisors when in need of assistance, additional trainings, or accommodations
Comply with mandated reporting including timely verbal and written notice to Child Welfare Services and documentation in organization's risk management software ADDITIONAL SPECIALIZED DUTIES BY PROGRAM AREA
Housing
Serve as lead for the Transitional Living Program and housing programs
Assist with housing navigation, search, and landlord recruitment efforts
Maintain and upkeep residential facilities and intake centers Outreach
Identify priority areas and strategies for successful street-based outreach to engage at-risk 18-24 years of age adults and perform outreach with staff at designated times
Operate Youth Coordinated Entry System checkpoints at various locations to quickly identify, assess, and connect youth in crisis to housing and services
Perform street-based outreach, walking/traveling specified routes to locate and engage with at-risk

Must be at least 21 years old
High School Diploma or equivalent
Bachelor's degree preferred
2 years of related work experience
Complete required certifications within 60 days of hire
Complete all required Branch and Position specific trainings as required within two weeks of hire
Ability to obtain a criminal record clearance
Maintain a valid California driver's license, active participation in the California DMV Pull Program, an acceptable driving record and approval status as determined by the Channel Islands YMCA and/or its insurance carrier
Health Screening Report and negative TB test required prior to first day of employment
