Qualifications

State License Willing to obtain is allowed.

Certifications BLS (AHA). Willing to obtain allowed for BLS (AHA).

Specialty Experience 1+ years of experience in Neurology in last 3 years.

EMR Must have experience with EMRs.

Skills Assessment NLG skills assessment with 2.7+ score.

Skills Assessment OPP skills assessment with 2.7+ score.

References 1 reference within the last 2 years. 1 Supervisor reference within the last 737 days.

Additional Confirmations I have not worked as a permanent employee of Lafayette General Health or an affiliate in the past year.

Additional Confirmations I confirm I have all the following experience: Epic EMR experience. Must have: Experience with general Ortho, Neuro, disability, and generalized weakness. Must have: Proficiency in Body Mechanics, Cervicalgia, Low back pain, Post-op cervical/spinal/shoulder repair, Spinal Conditions, and Total hip/knee replacements. Must have: Experience with Cognitive disorders, Multiple Sclerosis, Muscular dystrophy, Parkinson's disease, Post-Stroke Rehabilitation, Spinal cord injury, and Traumatic Brain injury.

COVID Vaccine 1 booster required. Medical and religious declinations are accepted. Willing to obtain allowed.

Flu Vaccine (Current Season) Must be for the current flu season. Medical and religious declinations are accepted. Willing to obtain allowed.

Travel Contract Local contracts within 65.0 miles not accepted.
