State License Willing to obtain is allowed.

Certifications ACLS. Willing to obtain allowed for ACLS.

Certifications PALS. Willing to obtain allowed for PALS.

Certifications NBRC (RRT) or NBRC (CRT)

Certifications BLS or BLS (AHA). Willing to obtain allowed for BLS or BLS (AHA).

Specialty Experience 1+ years of experience in General in last 3 years.

Specialty Experience 1+ years of experience in job's specialty.

EMR EPIC

Skills Assessment GRT skills assessment with 2.7+ score.

References 1 reference within the last 2 years.

Additional Confirmations I have submitted all time off requests. I am aware that I cannot make additional requests after submission.

Travel Contract Must travel at least 50.0 miles to receive a travel stipend. Local candidates are eligible, but the pay package may adjust the rate by a few dollars per hour, will not contain a travel stipend, and will be entirely taxable.

Requested Time Off Up to 6 days of time off can be requested.
