State License Willing to obtain is allowed.

Certifications BLS (AHA). Willing to obtain allowed for BLS (AHA).

Certifications ASHA (CCC-SLP)

Specialty Experience 1+ years of experience in job's specialty in last 3 years.

Skills Assessment PSP skills assessment with 2.7+ score.

References 1 Supervisor reference within the last 2 years. 1 reference within the last 2 years.

Additional Confirmations I will not be employed full time elsewhere during the assignment.

Additional Confirmations I am willing to float within a 25 mile radius of the facility.

Additional Confirmations I have not worked as a permanent employee of LifePoint Health in the past year.

Additional Confirmations I confirm I have all the following experience: Proficiency in EPIC EMR.
